Get rid of all our uses of virt_to_phys():
 1. Many were buggy, so the code needed fixing or clarifying anyway
 2. It means we can now experiment with kludging virt_to_phys() to be
    the same as virt_to_bus(), to get more buggy drivers working. I haven't
    included the kludge in this patch.
